All information will be mailed to the address you put on your application, unless you have submitted a complete Change of Address form.

If you change your name or address, or if either is incorrect on a notice you receive from DOP, notify DOP immediately. You may miss a job opportunity if you delay. For security reasons, we cannot accept a change of address by telephone. You may submit the change of address form by clicking here.

 

When contacting DOP by mail, please include the following information:

  • Name
  • Social Security Number (or ID number assigned by DOP)
  • New Mailing Address, including ZIP code
  • Municipality you live in (if different from mailing address)
  • Daytime telephone
  • Email address
  • Effective date of change

To receive an acknowledgement, please provide a self-addressed stamped envelope or a fax number.
